Submit Maximum Allowable Contribution (MAC) Worksheet
If you plan to take advantage of 15-year service-based or 50+ age-based catch-up contributions, you are required to complete Maximum Allowable Contribution (MAC) Worksheet. It is available as an attachment to SRA Form or it can be filed right here online. Please fill out the MAC worksheet and it will be submitted to your plan administrator.
